We look forward to treating you at our practice. As you are aware, allergy shots involve risk of potentially life-threatening reaction. Every allergist creates their allergy shots differently. In order to provide optimal care, and keep you safe during the allergy shot process, we require that you provide the following from your prescribing allergist prior to scheduling your initial appointment:
All clinical notes, testing and if you have asthma, your baseline spirometry data
“Recipe” (amount of each allergen in your injections) from the prescribing allergist
Injection treatment sheets (how far you are in the process of allergy shots)
You are responsible for either bringing or shipping your extracts to our office for your first visit
Have a plan for the future with your prescribing allergist: will they remake and release your extracts annually, or would you like our office to remake your extracts when they expire?
If the patient is under 18, and guardian will not be present for immunotherapy/visits:
Signed consent form for treatment of child under 18 years of age
Signed consent forms to provide immunotherapy to minor under 18 without adult present
If guardian cannot be physically present at the initial physician’s visit, they will need to join virtually by phone or video call
We must ensure your safety. Receiving the allergy injection at the initial visit in our office is at the discretion of the physician meeting you in our office.

Esperamos poder atenderle en nuestra práctica. Como usted sabe, las vacunas contra la alergia implican el riesgo de reacciones potencialmente mortales. Cada alergólogo crea sus vacunas contra la alergia de forma diferente. Para brindarle una atención óptima y mantenerlo seguro durante el proceso de la vacuna contra la alergia, requerimos que su alergista le proporcione lo siguiente antes de programar su cita inicial:

Usted es responsable de traer o enviar sus extractos a nuestra oficina para su primera visita.
Tenga un plan para el futuro con su alergólogo recetante: ¿rehacen y publicarán sus extractos anualmente, o le gustaría que nuestra oficina rehaga sus extractos cuando caduquen?
Si el paciente es menor de 18 años y su tutor no estará presente para la inmunoterapia/visitas:
Formulario de consentimiento firmado para el tratamiento de niños menores de 18 años.
Formularios de consentimiento firmados para proporcionar inmunoterapia a menores de 18 años sin la presencia de un adulto.
Si el tutor no puede estar físicamente presente en la visita inicial al médico, deberá unirse virtualmente por teléfono o videollamada.
Debemos garantizar su seguridad. Recibir las inyecciones contra la alergia en la visita inicial en nuestra oficina queda a discreción del médico que lo recibirá en nuestra oficina.
